Hospice

What is hospice?

Hospice is a philosophy of care that focuses on providing comfort and support to people facing life-limiting illness or injury as well as to their families.

Hospice patients are cared for in their home wherever the home may be (patient's home, home of relative of friend, or a nursing home).

Medicare and Medicaid cover the full cost of hospice care. Most private insurance covers hospice as well.

You should feel free to discuss hospice care at any time with your physician, other healthcare professional, clergy, family, or friends.

Hospice is a choice. A patient may choose to leave anytime. A patient may also be discharged if a patient shows signs of improvement, and the disease seems to be in remission.

Home Health Care

What is home health care?

Home health care is medical care and support provided in the comfort of a patient's home by licensed healthcare professionals.

We offer a range of services including skilled nursing, physical therapy, occupational therapy, speech therapy, home health aides, medical social workers, and spiritual counseling.

To qualify for home health care, a physician must determine that you need skilled care and that you are homebound.

Home health care provides medical care and support by licensed healthcare professionals, while home care provides non-medical support such as meal preparation, housekeeping, and companionship.

The frequency of visits will depend on your individual care plan and medical needs, which will be determined by your healthcare team.

Most insurance plans, including Medicare and Medicaid, cover home health care services. We will work with your insurance provider to determine your coverage and assist with any billing questions.

You can call us at (725) 206-5572 to schedule a consultation with our care team. We will work with you and your physician to develop a personalized care plan to meet your individual needs.
